Permission for School Administration of Non Prescription Medication 

Beginning with the 2013-14 school year, over-the-counter medications will no longer be 
administered at school without a written medical order (prescription). 
While parents/guardians may give their child over-the-counter medicines without a 
prescription, South Carolina laws require school nurses to have a medical order from a health 
care provider for all medications administered to students.


Permission for Medication

Medication to be 
given at school should be accompanied by this form, complete with the authorized prescriber’s signature.


Immunization Requirements for 2013-14

The following vaccinces are required:
 
Diphtheria, Tetanus and Pertussis
Tdap Booster
 Polio
Rubeola (Measles) 
Rubella 
(German Measles) 
Mumps
Hepatitis B
Varicella


Health Services:  School Exclusion List

A School Exclusion List indicates for which illnesses a child must be kept out of (excluded from) school. It also indicates when medical notes are needed for children to return to school after certain illnesses. Click here for more information.
